How to Order

PLEASE PRINT THIS ORDER FORM PAGE AND SEND A COPY WITH YOUR ORDER. KEEP A REFERENCE COPY FOR YOURSELF.

Name, complete address, contact numbers, best contact times and a 'needed by date' are required for each order placed. NOTE: I do not do photo alterations - photos are copied and transferred as is except for the size. If you send me a dark photo - it will be a dark transfer. If you send me a photo that is torn - the tear will show just as it does on your original. I only enlarge or reduce to fit into the design you've chosen. NOTE: Click on 'Policies' on the menu bar at the top of the page to get SHIPPING RATES to add to your order.

Ordering a Customized Photo Quilt is easy but takes a little bit of thought and planning:

1. Who is the quilt for - yourself or someone else?

2. How many photos do you want to include? ______________________

3. Which Custom Style do you prefer? _____________

Do you want the photos laid out in a specific order? if yes - did you label your photos (use only a post it or a felt tip marker)? Use the numbers that correspond to the design style you chose.

4. What should be main color and contrast color be? If it is for someone else - find out if they have a favorite color? or if you think they might hang it in their living room - what color is their living room? i.e. dark blue/light blue; burgundy & white; COLOR CHOICES ARE REQUIRED along with a COLOR SAMPLE! The sample is especially important so that I can select fabric that matches the color you really want.

5. Date you need the quilt in your hands? Please allow 4 to 6 weeks for delivery during non-holiday times.
